Late Arrivals
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session. Out of respect and consideration to your therapist and other customers, please plan accordingly and be on time.
If I experience pain or discomfort during the session, I will immediately inform my therapist so that pressure can be adjusted to my level of comfort. I will not hold my therapist responsible for any pain or discomfort I experience during or after the session.
I understand that the services offered today are not a substitute for medical care. I understand that my therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness.
I affirm that I have notified my therapist of all known medical conditions, injuries and allergies.
I agree to inform the therapist of any changes in my health and medical condition or if I become pregnant! I understand that there shall be no liability on the therapist’s part should I forget to do so.
I understand that massage is entirely therapeutic and non-sexual in nature.
I agree to reschedule an appointment if I am ill. Meaning if I have had a FEVER in the last 24 hrs. as well as if I am COUGHING, SNEEZING, experiencing NEW or ADDITIONAL SHORTNESS OF BREATH, NAUSEA OR VOMITING, DIARRHEA, SORE THROAT. (Please note that this INCLUDES a chronic cough from COPD, etc. If you are coughing for any reason, you are at risk of spreading illness. I apologize for any inconvenience.) If I show up to an appointment, with these symptoms, I will be asked to reschedule.
I will wash my hands and use hand sanitizer when prompted inside the office.
I agree to notify my Massage Therapist immediately if I feel my health is at risk in the Massage Therapy environment, for any reason, to allow for it to be addressed by the Massage Therapist.
Cancellation/Rescheduling Policy:
I understand that if I do not show up for my scheduled appointment I will be charged full price for the session and will not be permitted to schedule again unless paid. I give permission to be charged for the missed appointment with my Credit Card on file. If I prepaid for the missed session, no refund will be given.
I understand that I am allowed to reschedule a massage for any reason without penalty! I will simply be charged for half of the massage that week which will then become a credit toward my next booked massage. Credits have no monetary value and will not be converted back to money for refunds.
I understand that if I choose to cancel my massage with less than 5 days of notice, instead of rescheduling, I will be charged a fee of 50% of the service price. I understand the Credit Card I have on file will be charged or I must make arrangements to pay this fee within 7 days. (Exceptions may be made if I have recurring appointments set up for every week, every 2 weeks or every 3 weeks and the reason for cancellation is because rescheduling was not possible or impractical. The final decision will be made by business owner.)
By signing this release, I hereby waive and release my therapist from any and all liability, past, present, and future relating to massage therapy and bodywork
